TriState EyeCare Associates is proud to announce a new service to its current patients with prescriptions for contact lenses. Order your contact lens replacements via email. This is NOT a secure line transaction so please do not submit your credit card number. This service requires that our office have your credit card number on file. You may, at any time, call the office to place a card number on file to expedite future ordering.


To order contact lenses via email request, please fill in all fields, then press SUBMIT.
